All
requests for data are reviewed by the Data Utilization Committee
for feasibility, appropriate use of data, and priority. Prior
to requesting data, please read the CONFIDENTIALITY
POLICIES AND PROCEDURES FOR DATA MANAGEMENT (DOC form).
To request data, complete the
SFMR Data Request Form (DOC form) and send it to Dr. Karla
Kerlikowske, chair of the Data Utilization Committee. The
data dictionary (PDF form) that describes variables in the
SFMR and the data collection forms (pdf
questionnaire and .doc
of radiology variables) are available for review.
Priority of data use is as follows:
1. UCSF investigators
2. non-UCSF academic investigators
IRB approval is required for all data use.
Investigators using SFMR data are expected to submit a timely
report on data use. All publications resulting from data analysis
must acknowledge funding sources for the SFMR.
